Welcome to the 2021-2022 school year at SPOT 127! We are very happy to have your son/daughter in our ONLINE digital media program. We also appreciate the commitment you have made. Parent involvement and communication are very important to us. Our hope is that your son/daughter will consider their experience at SPOT 127 a great opportunity to learn digital media. Our goal is to provide each student the tools to tell the stories that matter most in their lives using audio, video and photography. 

We will update you about special events, community partnerships and important dates through our monthly SPOT 127 email newsletter.  

This form contains several agreements SPOT 127 participants and their parents need to review and sign to be part of the program. These agreements are required because we are part of Rio Salado College and are governed by Maricopa County Community Colleges District. The agreements include a Student Behavior Notice, Medical Emergency Info, Photo Release, Liability Release, Student Artist Consignment Agreement, and Technology Agreement. By signing off on these agreements, you agree to follow the policies. Please electronically fill-in and digitally sign all agreements within this form by or before Wednesday, September 8, 2021. NOTE: This form is for the following session: Fall 2021

Finally, there are several ways that you and other community members can financially support SPOT 127:

  • SPOT 127 is a partner in the Fry’s Food Stores Community Rewards Program. Our organization number is 39042 (Friends of Public Radio Arizona/SPOT 127).
  • SPOT 127 is also registered with Amazon Smile. When you shop at Amazon your purchase automatically helps SPOT 127. Visit smile.amazon.com to sign up (Friends of Public Radio Arizona – KJZZ/KBACH).
  • Support the students at SPOT 127 through a direct donation. We also qualify for the Arizona Charitable Tax Credit.

Behavior Agreement
  1. To avoid liability and provide a safe environment where students do not engage inappropriate behavior, students will be closely supervised in an online, remote capacity during class meetings.
  2. All Local High School Districts and Rio Salado College rules and regulations apply. Students with or in the presence of illegal substances will be asked to contact parent for immediate removal.
  3. No smoking is allowed. This includes e-cigarettes and alternate forms of nicotine use.
  4. Any student damaging property will be charged for it, and notices will be sent to parents. SPOT127 is not responsible for loss of personal items or any damages related to online participation
Medical Release
Answering this question does not affect the outcome of your application. Information about family income allows us to apply for special funding.
